Nurse consultation

What is a nurse?

A general nurse is a healthcare professional who provides patient care – participates in the treatment process, manages patient care, educates patients about health issues, plans and organises care in collaboration with other members of the clinic team.

Nursing consultations are particularly important in the care of chronic patients, patients in the post-hospitalisation phase, in order to assess the patient’s health status and the care needs. Chronic patient care means a long-term collaboration with a patient who has been diagnosed with a chronic condition such as diabetes, chronic heart disease, chronic kidney disease.

How does a nurse consultation work?

During a nurse consultation, the patient is assessed and the patient’s medical history and presenting complaints are discussed. Vital signs such as blood pressure, pulse, respiration are taken. The patient’s weight, height, abdominal girth, body mass index (BMI) and blood sugar are also taken. Assessing adherence to treatment and care, including medication habits, regular blood pressure monitoring, glucose control, healthy diet and moderate physical activity, is important for quality patient care and monitoring in chronic diseases.

At the end of the consultation, in collaboration with the patient, a care plan is drawn up for the individual care of the patient according to his/her physical and medical needs, which includes defining the problem or diagnosis of care, the actions to be taken, evaluation of the results and, if necessary, adjustments to the plan drawn up.
